Release of Claims and Hold Harmless Agreement
In consideration of your acceptance of this registration, I hereby for myself, my heirs, my executor and my administrators, wave and release any and all rights and claims for damages, theft, injury, and/or death which I may have against Ishmael’s Blessing (IB), Global Institute or Ministry and Training (GIMT), Reach International Church, Praise Cathedral, Erie Church of God, or coordinating groups and any individuals associated with an IB-sponsored ministry, project, or related activity, and agree to hold harmless said parties from liability therefore. This release and hold harmless includes damages, theft, injury and/or death, which may occur during, in preparation for, or in transit to and from said event. This waiver of liability applies to any negligent act or omission and any intentional act intended to protect my safety and well-being. I also give permission for free use of my name, photograph and likeness in any broadcast, telecast or printed media of said event. This waiver is signed by me (or my guardian if under age 18) in the interest of permitting IB to exist and to serve the missions community.
